At Meridian Bank, we understand that balancing the demands of work and life can make it challenging to maintain a healthy lifestyle. We value you and are committed to supporting your well-being by providing the tools and resources you need to thrive. To help you achieve this, we’re offering a wellness program with an opportunity to save on your per pay medical/prescription drug contribution. ​​
​The participant is responsible for submitting the Physician Results Form as part of the wellness program. This form must be returned to Wellworks For You and can be found on the Wellworks Portal.
​​​​PLEASE NOTE: If your spouse/domestic partner is enrolled in Meridian Bank’s medical/prescription drug plan (including Davis Vision), both the employee and spouse/domestic partner must comply to be participants in this program.
REMEMBER: Preventive care is covered 100% when seeing an in-network provider.

Participation in this program is voluntary.

Wellness Program Eligibility
To be eligible for savings in the current 2025-2026 plan year:

Employees and their spouse/domestic partner enrolled in a Meridian Bank medical/prescription drug plan (including Davis Vision) who complete their annual physical and biometric screening between July 1, 2024, and September 1, 2025, will be eligible for savings beginning July 1, 2025.

You must submit proof of completion via the Wellworks For You member portal by September 1, 2025. Please see the Benefits Guide for more information.

​If proof of completion is not submitted by September 1, 2025, Meridian Bank will increase your monthly medical/prescription drug (including Davis Vision) contributions as follows:

Employee Only - $25.00

* Employee & Spouse/Domestic Partner - $50.00

To be eligible to continue seeing savings in the 2026-2027 plan year:

Employees and their spouse/domestic partner enrolled in a Meridian Bank medical/prescription drug plan (including Davis Vision) who complete their annual physical and biometric screening between July 1, 2025, and June 30, 2026, will be eligible for savings beginning July 1, 2026.

You must submit proof of completion via the Wellworks For You member portal by June 30, 2026. Please see the Benefits Guide for more information.
* Important Note: If you have a spouse/domestic partner as a covered dependent, both the employee and spouse/domestic partner must complete their annual physical and biometric screening to qualify for savings.
