REMEMBER: You MUST complete the Spousal/Domestic Partner Affidavit below and send to HR upon enrollment of benefits. If not received by your benefits effective date, and your spouse/domestic partner is enrolled in medical/prescription drug (including Davis Vision) coverage, the $34.62 per pay surcharge will automatically be applied to your per pay contribution each pay period until this form is received and reviewed by HR.
