As a plan participant, you are entitled to receive various notices and disclosures, including a comprehensive description of your rights and obligations under the Meridian Bank Health and Welfare Plan. A copy of these notices and disclosures and the most recent Plan Documents for our plan below.

 

You will be able to view the notices and download and print copies. In order to ensure that you fully understand the benefits available to you and your obligations as a plan participant, it is imperative that you familiarize yourself with the information contained within these notices and the Plan Documents and share them with you covered family members. If you would like to receive a paper copy of the Plan Document or any of the posted notices, you may contact Human Resources.
